What we collect, why, how long we keep it and how to ask us about it.
What this website collects
When you use the request form we collect your name, phone number, email address, your postcode, what the request is about and who you would like to see, and anything you write in the message box. Please do not put clinical detail in the message; reception only needs enough to book the right appointment. The form also records the time it was sent and your IP address, used only to block automated submissions.
The same-day request form collects your name, date of birth and phone number so that the duty clinician can find your record and ring you back. Nothing you enter on this site is added to your medical record until a member of staff has read it.
Why we use it
- To book, change and remind you of appointments, and to reply to your request (provision of NHS healthcare, which is a public task).
- To pass a same-day request to the duty clinician the same morning (provision of healthcare).
- To send appointment reminders and invitations for reviews, screening and vaccinations by text, if you have given a mobile number (public task; reply STOP to end texts).
- We do not use patient photographs on this site; the images here are of staff and models.
How long we keep it
Website requests are kept for three months after they are answered and then deleted; anything that becomes an appointment or a clinical note is kept as part of your medical record under the NHS Wales records retention schedule. Reminder lists are kept while you are registered with us.
Who sees it
Reception and the clinicians, the text-message service that sends confirmations and reminders (name, phone number and appointment time only), and the NHS Wales systems that hold your record and send prescriptions to your pharmacy. The request form is delivered by email to the practice; we do not use third-party form services or marketing platforms, and we never sell or share your details.
Cookies
This site sets a single session cookie to limit repeat form submissions. It contains no personal data and expires when you close the browser. The embedded map on the find-us and request pages is served by Google and may set cookies under Google's own policy.
Your rights
You can ask for a copy of what we hold, ask us to correct or delete it, or object to how we use it. Email enquiries.llandafffields@wales.nhs.uk, write to the practice manager at the address above, or ask reception for a subject access request form. You can also complain to the Information Commissioner's Office at ico.org.uk.
