Referrals

Physios and GPs we work with.

Six of the practices around Bridgend that refer clients to us and get updates back, and how it works if you are a clinician thinking of referring. Names on this demonstration site are fictional.

Not on the list? We work with any physio, GP or surgeon; bring their letter. If nobody has assessed your injury yet, any of these can, or your own GP.

How referral works

Four steps, one plan.

  1. Assessment

A physio, GP or surgeon assesses the injury and decides it is ready for rehab training.

  2. The notes

You bring their letter, or with your consent we ask for it: findings, limits and goals.

  3. Staged training

We train you from the right stage, to their limits, with tests to move on.

  4. Updates back

Every four weeks a short update goes back; sooner if anything changes or worries us.

For clinicians

Referring a patient to us.

Send a short letter with the diagnosis, any restrictions and what you want built, or ask the patient to bring it. We will see them within a week, start them at the stage your findings suggest, and send you a one-page update every four weeks with the test results. If symptoms change or anything does not fit, we send them straight back to you.

  • Seen within a week, weekdays from 7am
  • Your limits followed; we never diagnose or treat
  • One-page updates with test results
Refer or book

After the physio, the long middle. We do that bit.

Book a first session: an hour, £55, with your physio's or GP's notes, simple tests, your stage and a written plan. If nobody has assessed the injury yet, we will tell you who can, and the referrals page lists practices we work with.
