What we collect, why, how long we keep it and how to ask us about it.

What we collect

When you use the booking form we collect your name, phone number, email address and postcode, what you need, when you can come, a preferred or surgery date and anything you write. The form also records the time it was sent and your IP address, used only to block automated submissions. The stage checker works on the page from what you choose and stores nothing.

As a client we hold your health history, the notes or letters from your physio, GP or surgeon, test results, session notes, consent records and payments. Health information is special category data, held with your explicit consent and handled as such.

Why we use it

  • To plan and run your rehab (our contract with you, and your explicit consent for health information).
  • To send short updates to your physio, GP or surgeon, only with your consent.
  • To provide receipts and notes for an insurance claim, at your request.
  • To keep accounting records (a legal obligation).

Who sees it

Owain and Ffion. Your physio, GP or surgeon receives updates only if you agree, and you can withdraw that at any time. Our booking, notes and email providers process it for us. We never sell or share your details for marketing, and we never contact your employer.

How long we keep it

Enquiries that do not become sessions are deleted after six months. Health information, notes and test results are kept for eight years after your last session, in line with guidance for health-related records, then securely deleted; accounting records for six years.

Cookies

This site sets one session cookie when you send the booking form, to limit repeat submissions; it expires when you close the browser. There are no analytics or advertising cookies. The map on the find-us page is loaded from Google, which may set its own cookies when it is shown.

Your rights

You can ask for a copy of what we hold, ask us to correct or delete it, object to how we use it, or withdraw consent (in which case we will have to stop training you, and we will say so). Email hello@backontrackrehabpt.co.uk or write to the address above. You can also complain to the Information Commissioner's Office at ico.org.uk.
