What we collect, why, how long we keep it and how to ask us about it.
What we collect
When you use the form on this site we collect your name, phone number, email address and postcode, what you are asking about, the date you mention if any, and anything you write in the message box. The form also records the time it was sent and your IP address, used only to block automated submissions.
If you or your child become a patient we keep a clinical record: case history, assessment results, session notes, reports and letters to and from GPs, schools and other professionals, all with your consent. Health records are special category data and are held under the stricter rules that apply to them. Children's records have particular protection. We do not photograph or film without separate written consent. The milestones page stores nothing: the age you type is only in the web address.
Why we use it
- To book and remind you of appointments (the steps needed before and under a contract).
- To assess and treat safely, which includes keeping clinical records (contract, and our obligations as HCPC registrants).
- To write to your GP, school or other professionals with your agreement.
- To share information with social services or the police without consent where a child or adult is at risk, as the Wales Safeguarding Procedures require (legal obligation).
How long we keep it
Enquiries that do not become appointments are deleted after six months. Adult records are kept for eight years after your last visit. Children's records are kept until they are twenty-five, or twenty-six if they were seventeen when treatment ended.
Who sees it
Siân and Matthew; schools, GPs and others with your agreement; and our accountant for invoice records. The form is delivered by email to us; we do not use third-party form services or marketing platforms. We do not sell or share your details for marketing.
